ZT-01 glucagon-response restorer
Zucara Therapeutics
What it is
A once-daily investigational therapy intended to restore the body's own glucagon counter-regulation during hypoglycemia by blocking pancreatic somatostatin receptor 2. Phase 2 ZONE is testing nocturnal hypoglycemia prevention in T1D.

Evidence behind this assessment
Key evidence notes. Study results, product eligibility and access answer different questions.
- Who was studied?
- Study populations and analysis groups vary. Product age limits alone do not describe who was studied.See the linked studies and their populations →
- Benefit or performance
- Time in range: The goal is fewer lows rather than higher TIR; phase 2 will show whether hypoglycemia prevention improves usable glucose range.[3]
- Important harms and treatment burden
- Read the safety discussion and original sources. A missing summary does not establish safety.
- Approval and country access
- Country-specific approval and access are not summarized in this record.Approval, trial recruitment, local supply and funding are separate. Check the cited label or access source.
- Follow-up and remaining uncertainty
- Read the full discussion and original sources for follow-up duration and study limitations.
Research status alone does not establish approval, clinical benefit or local availability. This record cites company or conference reports. Those sources may describe interim findings; their source type is shown in the source list.
Editorial score: calculation and evidence
A weighted editorial judgment on a 0–100 scale, not a probability of success or a measured treatment effect. Higher criterion scores mean more favorable assessments.
Default calculation: 55 × 25 + 30 × 20 + 10 × 15 + 35 × 20 + 55 × 10 + 5 × 10 = 3425; divide by total weight 100. Unrounded weighted result: 34.25.
A phase 1b crossover clamp study in 24 randomized adults with T1D (Abitbol et al., Diabetologia 2026) showed increased glucagon responsiveness during insulin-induced hypoglycemia; phase 2 ZONE is testing nocturnal hypo frequency.[4]
The goal is fewer lows rather than higher TIR; phase 2 will show whether hypoglycemia prevention improves usable glucose range.[3]
ZT-01 is not an automated delivery system; it supports counter-regulation rather than dosing decisions.[1]
Zucara describes a once-daily therapeutic, positioned as less burdensome than carrying/using rescue glucagon, but it is still investigational.[2]
As with AID systems, glycemic criteria reflect the levels achieved in real-world or trial Type 1 use rather than the improvement over baseline, and Type 2 diabetes data is not used to score a Type 1 system.
The full picture
ZT-01 is not a bihormonal pump, but it belongs near the bihormonal frontier because it tries to give back the missing second-hormone defense against lows. In type 1 diabetes, glucagon release during hypoglycemia is often blunted. ZT-01 blocks somatostatin receptor 2 in the pancreas, aiming to let endogenous glucagon rise when glucose falls. If phase 2 shows fewer nocturnal lows, it could complement AID systems rather than replace them.
